Even baking bread or making a smoothie in the blender are forms of processing. But ultra-processed food (UPF) is different, says van Tulleken in <em><a href=\"https:\/\/www.shortform.com\/app\/book\/ultra-processed-people\/preview\">Ultra-Processed People<\/a><\/em>. Ultra-processing doesn\u2019t just modify food; it transforms food into something else entirely.<\/p>\n\n\n\n<p>Van Tulleken argues that <strong>UPF isn\u2019t really food: It\u2019s an industrially produced substance that looks, smells, and tastes like food.<\/strong> It starts out as food\u2014usually high-yield crops like corn or soybeans that are grown for animal feed and, as a result, are subsidized in many countries. These inexpensive crops are reduced to their basic molecular constituents, then chemically and physically altered to form synthetic molecules that can mimic all kinds of natural ingredients. These synthetic molecules are then reassembled into something that looks like food and are filled with additives that add flavor, color, saltiness, and sweetness. <strong>Without the additives, UPF probably wouldn\u2019t taste like food at all.<\/strong>&nbsp;<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table class=\"has-fixed-layout\"><tbody><tr><td><strong>How Corn Becomes UPF<\/strong><br><br>In <a href=\"https:\/\/www.shortform.com\/app\/book\/the-omnivore-s-dilemma\" rel=\"nofollow\"><em>The Omnivore\u2019s Dilemma<\/em><\/a>, science journalist and author Michael Pollan uses the example of corn to explain how a few cheap crops form the basis for so much UPF. He explains that <a href=\"https:\/\/www.shortform.com\/app\/book\/the-omnivore-s-dilemma\/part-1-1\" rel=\"nofollow\">out of approximately 45,000 items in an average American supermarket, a quarter contain corn<\/a>. It\u2019s in beer, cake mix, frozen meals, and most meat, to name just a few.<br><br>Corn is so ubiquitous because, since the 1970s, US government policies influenced by business interests have <a href=\"https:\/\/www.shortform.com\/app\/book\/the-omnivore-s-dilemma\/part-1-2\" rel=\"nofollow\">driven down the price of corn while simultaneously pushing farmers to consolidate into larger farms<\/a>. Because most farms grow a single crop, the only way for them to make enough money to survive is to grow more corn. This effectively subsidizes buyers of cheap corn, like Coca-Cola and Cargill, who can make an even greater profit by transforming it into UPF.&nbsp;&nbsp;<br><br>UPF corn chips are a simple example of <a href=\"https:\/\/www.washingtonpost.com\/wellness\/2023\/06\/27\/ultra-processed-foods-predigested-health-risks\/\" target=\"_blank\" rel=\"noreferrer noopener\">how corn becomes UPF<\/a>. First, corn is cooked, soaked, and pulverized into a dough. Then, it\u2019s fed into extrusion machines, which are used throughout the food industry to mass-produce all types of UPF. The extrusion machine applies intense heat and pressure to the dough, destroying the cell walls inside the corn. Rotating screws blend and force the dough through a small hole in the machine. Next, the dough is molded into sheets, flattened, and cut into chip shapes. The chips are baked and deep-fried. Finally, the chips are coated with artificial flavors and colors, as well as salt, sugar, and MSG.<br><br>A similar method can be used to convert a variety of cheap crops into a flavorless \u201cextrudate\u201d that can then be molded into any shape and flavored with any additives to create thousands of UPF products.<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p>Making \u201cfood\u201d from industrially created ingredients is not only cheaper than making it from natural ingredients, but it also lengthens the food\u2019s shelf life and makes centralized distribution easier, which further contributes to UPF\u2019s low price. The lower cost of UPF makes it more desirable to consumers; the lower cost of UPF\u2019s synthetic ingredients maximizes profits for food corporations.<\/p>\n\n\n\n<p>(Shortform note: Pollan further explains how UPF companies maximize profit. He says that processing not only gets people to buy more food, it also <a href=\"https:\/\/www.shortform.com\/app\/book\/the-omnivore-s-dilemma\/part-1-4#increasing-profits\" rel=\"nofollow\">allows companies to capture more of the money a consumer spends on food<\/a>. For example, if you spend a dollar on eggs, 40 cents goes back to the farmer, but if you spend a dollar on corn sweetener, only 4 cents goes to the farmer, and UPF companies get the rest.)&nbsp;&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-classifying-upfs\">Classifying UPFs<\/h3>\n\n\n\n<p>Van Tulleken cites Brazilian scientist Carlos Monteiro, who first described the concept of \u201cultra-processed food\u201d in 2009. Monteiro devised the NOVA system, a now widely used food classification system that divides food into four groups based on the level of processing:<\/p>\n\n\n\n<p><strong>1. Unprocessed or minimally processed food.<\/strong> These are foods in their natural state. Examples include fruits, vegetables, and meat.&nbsp;<\/p>\n\n\n\n<p><strong>2.<\/strong> <strong>Processed culinary ingredients.<\/strong> These are ingredients that we use in traditional cooking by combining them with foods in the first category. Examples include butter, sugar, salt, oil, and vinegar.&nbsp;<\/p>\n\n\n\n<p><strong>3.<\/strong> <strong>Processed foods.<\/strong> These are foods that are processed mainly for preservation and sold already prepared to eat. Examples include canned food, smoked fish, and fresh bread.&nbsp;<\/p>\n\n\n\n<p><strong>4.<\/strong> <strong>Ultra-processed foods.<\/strong> These are foods <strong>made primarily from industrially created ingredients<\/strong>, using complex industrial processes such as extracting substances from whole foods and chemically modifying those substances. These foods are intended to be highly profitable, convenient, and desirable. Examples include fast food, frozen meals, most packaged snacks, most store-bought bread, and soda.<\/p>\n\n\n\n<p>(Shortform note: Critics of the NOVA system argue that it\u2019s too vague to be useful to either consumers or researchers. They contend that even experts have difficulty determining which foods should belong to which group; this leads to <a href=\"https:\/\/www.nature.com\/articles\/s41430-022-01099-1\" target=\"_blank\" rel=\"noreferrer noopener\">inconsistent classifications<\/a>. However, research also shows that the <a href=\"https:\/\/www.foodnavigator.com\/Article\/2023\/07\/28\/nova-classification-matches-consumer-instincts-study-finds\" target=\"_blank\" rel=\"noreferrer noopener\">NOVA system is intuitive to consumers<\/a>: People\u2019s perceptions of foods and their processing levels generally line up with the foods\u2019 NOVA classifications.)&nbsp;<\/p>\n\n\n\n<p>If you\u2019re wondering if there\u2019s an easy way to identify UPF, van Tulleken says it\u2019s usually anything that\u2019s wrapped in plastic and contains at least one ingredient that you wouldn\u2019t find in your kitchen. He also suggests that low-fat, no-fat, and other \u201cdiet\u201d foods are almost always UPF.&nbsp;<\/p>\n\n\n\n<p>(Shortform note: Van Tulleken\u2019s ideas on packaged low-fat foods echo Pollan\u2019s contention in his 2007 article, \u201c<a href=\"https:\/\/www.nytimes.com\/2007\/01\/28\/magazine\/28nutritionism.t.html\" target=\"_blank\" rel=\"noreferrer noopener\">Unhappy Meals<\/a>\u201d (which influenced Monteiro\u2019s thinking in formulating the NOVA system) that any food that comes in a package advertising health benefits isn\u2019t really food\u2014and isn\u2019t really healthy at all. Most experts, including van Tulleken and Pollan, agree that low-fat foods don\u2019t help you lose weight, and generally result in weight gain and other health problems. Pollan explains why in his book <a href=\"https:\/\/www.shortform.com\/app\/book\/in-defense-of-food\" rel=\"nofollow\"><em>In Defense of Food<\/em><\/a>: In low-fat foods, UPF companies replace saturated fats with hydrogenated seed oils. The <a href=\"https:\/\/www.shortform.com\/app\/book\/in-defense-of-food#the-low-fat-fiasco\" rel=\"nofollow\">process of solidifying vegetable oil using hydrogen creates trans fats<\/a>, which are now known to cause heart disease and high cholesterol.)&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-why-we-rely-on-ultra-processed-foods\"><strong>Why We Rely on Ultra-Processed Foods<\/strong><\/h3>\n\n\n\n<p>If we know they\u2019re not healthy, why do ultra-processed foods have such a powerful hold on American diets? Experts say <strong>the appeal of these foods starts with their convenience<\/strong>. These foods offer <a href=\"https:\/\/www.theguardian.com\/environment\/article\/2024\/may\/22\/eliminating-ultra-processed-foods-pros-cons\" target=\"_blank\" rel=\"noreferrer noopener\">what many busy families need<\/a>: they\u2019re quick, reliable foods that require minimal preparation and won\u2019t spoil before you can get around to using them. For many households, the option to heat up a frozen dinner, an instant soup, or a box of macaroni and cheese can mean the difference between eating a hot meal and not eating a proper dinner at all.<\/p>\n\n\n\n<p><strong>Cost is another crucial factor<\/strong>. In many communities, especially in \u201cfood deserts\u201d where access to fresh foods is limited, ultra-processed foods often represent <a href=\"https:\/\/www.theatlantic.com\/ideas\/archive\/2023\/05\/ultra-processed-food-chris-van-tulleken\/674101\/\" target=\"_blank\" rel=\"noreferrer noopener\">the most affordable way to feed a family<\/a>. When you\u2019re stretching a tight budget, the long shelf life of these products also means less food waste. And it means fewer trips to the grocery store than if everything you bought were fresh and had a short shelf life.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-what-are-the-negative-health-effects-of-ultra-processed-food\"><strong>What Are the Negative Health Effects of Ultra-Processed Food?<\/strong><\/h2>\n\n\n\n<p>Ultra-processed foods are often loaded with saturated fat, salt, and sugar. Yet they contain few of the nutrients our bodies need. Worse yet, when these foods dominate our diets, they tend to squeeze out more nutritious options. The health implications are sobering. Research has linked regular consumption of ultra-processed foods to an array of serious conditions:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Heart health<\/strong>: Studies show increased risks of <a href=\"https:\/\/www.vox.com\/science-and-health\/2019\/6\/11\/18652653\/diet-weight-loss-ultra-processed-foods-microbiome\" target=\"_blank\" rel=\"noreferrer noopener\">high blood pressure, cardiovascular disease, heart attacks, and strokes<\/a>.<\/li>\n\n\n\n<li><strong>Metabolic issues<\/strong>: Higher rates of <a href=\"https:\/\/www.sir.advancedleadership.harvard.edu\/articles\/stem-tsunami-suffering-metabolic-disease-limit-ultra-processed-foods\" target=\"_blank\" rel=\"noreferrer noopener\">obesity and Type 2 diabetes<\/a> have been linked to the consumption of ultra-processed foods.<\/li>\n\n\n\n<li><strong>Cancer risk<\/strong>: Certain <a href=\"https:\/\/cancerblog.mayoclinic.org\/2024\/02\/06\/is-there-a-connection-between-ultraprocessed-food-and-cancer\/\" target=\"_blank\" rel=\"noreferrer noopener\">types of cancer<\/a> appear more frequently in heavy consumers of these foods.<\/li>\n\n\n\n<li><strong>Mental health<\/strong>: Ultra-processed foods have also been linked to <a href=\"https:\/\/www.ama-assn.org\/delivering-care\/public-health\/what-doctors-wish-patients-knew-about-ultraprocessed-foods\" target=\"_blank\" rel=\"noreferrer noopener\">increased rates of depression<\/a> and other mental health disorders.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-upfs-are-linked-to-obesity\"><strong>UPFs Are Linked to Obesity<\/strong><\/h3>\n\n\n\n<p>Studies have indicated that it\u2019s the processing of UPF that causes these health problems, rather than simply the nutritional content. In <em>Ultra-Processed People<\/em>, Van Tulleken mostly focuses on weight gain and obesity because there are more studies of UPF\u2019s effects on weight.<\/p>\n\n\n\n<p>Widespread obesity didn\u2019t exist for much of human history, writes van Tulleken. Until the beginning of the 20th century, obesity was very rare, especially in children. Since the 1970s, however, obesity has skyrocketed, and it affects more children than ever. Not coincidentally, we also began eating increasing amounts of UPF in the 1950s.<\/p>\n\n\n\n<p>(Shortform note: Scientists often refer to the precipitous rise in obesity throughout the Western world since the 1970s as an \u201cobesity epidemic.\u201d In 1976, the prevalence of obesity in American adults was 15%; <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC9611578\/\" target=\"_blank\" rel=\"noreferrer noopener\">by 2000, that number had doubled to 30.9%<\/a>. As of 2020, <a href=\"https:\/\/www.cdc.gov\/obesity\/data\/adult.html\" target=\"_blank\" rel=\"noreferrer noopener\">it was 40.9%<\/a>. Worldwide obesity <a href=\"https:\/\/www.who.int\/news-room\/fact-sheets\/detail\/obesity-and-overweight\" target=\"_blank\" rel=\"noreferrer noopener\">more than doubled between 1990 and 2022<\/a>. In American children, obesity rates have tripled over the last three decades; <a href=\"https:\/\/www.hsph.harvard.edu\/obesity-prevention-source\/obesity-trends-original\/global-obesity-trends-in-children\/\" target=\"_blank\" rel=\"noreferrer noopener\">one out of every six kids is obese<\/a>. Doctors generally screen for obesity using <a href=\"https:\/\/www.cdc.gov\/obesity\/basics\/adult-defining.html\" target=\"_blank\" rel=\"noreferrer noopener\">Body Mass Index<\/a> (BMI), which is weight in kilograms divided by the square of height in meters. A BMI of 30 or higher is considered obese. However, BMI merely helps evaluate possible risks; it\u2019s not a diagnosis of an individual\u2019s actual health status.)<\/p>\n\n\n\n<p>Experts had long thought that the reason obesity wasn\u2019t a problem until recently was historical food scarcity, often resulting from famines and food shortages. However, <strong>more and more studies now demonstrate that UPF is probably the primary cause of the global increase in obesity<\/strong>. One study showed that people on a primarily UPF diet consumed an average of 500 calories more per day (and gained weight accordingly) than people on a non-UPF diet, even though both diets contained identical amounts of fat, salt, sugar, and fiber. The people on the non-UPF diet actually lost weight.<\/p>\n\n\n\n<p>(Shortform note: While van Tulleken says that UPF leads to various serious diseases, including obesity, he doesn\u2019t explain what makes obesity unhealthy on its own. Studies have consistently demonstrated that <a href=\"https:\/\/www.hsph.harvard.edu\/obesity-prevention-source\/obesity-consequences\/health-effects\/\" target=\"_blank\" rel=\"noreferrer noopener\">obesity increases the risk of diabetes, heart disease, arthritis, depression, and some cancers<\/a>. But newer research also indicates that <a href=\"https:\/\/www.science.org\/content\/article\/obesity-doesn-t-always-mean-ill-health-here-s-what-scientists-are-learning\" target=\"_blank\" rel=\"noreferrer noopener\">just because someone is overweight or obese, it doesn\u2019t necessarily mean they\u2019re unhealthy<\/a>. These studies show that people with too much visceral fat\u2014fat located deep in the abdomen\u2014have a higher risk of health problems. But excess subcutaneous fat\u2014fat under the skin of the thighs, arms, and back\u2014may <em>promote<\/em> good health. In short, some people are resistant to the negative effects of obesity.
